Report: "Rust" AD had history of unsafe working conditions

(Santa Fe, NM) -- More details are emerging about the deadly shooting on the set of an Alec Baldwin western being filmed in New Mexico.  

The assistant director of the film production reportedly had a history of allowing unsafe working conditions. Court records show he was the one who handed Baldwin the gun on the set of "Rust" before last week's shooting that left cinematographer Halyna Hutchins, incorrectly indicating the gun was "cold," or didn't contain live ammo. Director Joel Souza was also injured but treated and released from the hospital.

A candlelight vigil was held Sunday night at the International Alliance of Theatrical Stage Employees local union headquarters in Burbank, California.
